Abstract
BACKGROUND: The Missouri Quality Improvement (MOQI) initiative is one of seven sites funded by the Centers for Medicare and Medicaid as part of the Initiative to Reduce Avoidable Hospitalizations Among Nursing Facility Residents. MOQI partners with 16 St. Louis area nursing homes to reduce avoidable hospital admissions/readmissions and cost, and improve health outcomes. The initiative employs Advanced Practice Registered Nurses in each home, and supports improved healthcare delivery through the use of multidisciplinary quality improvement teams, enhanced health information technology and advanced directives, INTERACT tools, and feedback reports.
PURPOSE/METHODS: To report on the root cause analysis data of 3,668 nursing home resident hospitalizations occurring over nearly three years. This is a mixed method descriptive study of the root cause analysis INTERACT 3.0 tool entered into a Qualtrics electronic survey platform.
Speaker profile
An internationally recognized nursing informatician and clinical expert with 25+ years of research and clinical practice leadership, Dr. Alexander’s research informs professional societies, organizations, and prestigious peer reviewed journals about new models of care including technology implementation trends and safety and quality impacts, especially in long term care.
He leads an AHRQ-funded R01 and co-leads two CMS national demonstration projects in the United States.
