About the seminar

Chemotherapy treatments for children are high risk with rich interdependencies and protocol mandated care.

Electronic medication management systems that semi-automate chemotherapy protocols can contribute to a reduction of errors and unwanted deviations from protocols.  However, they also have limitations and may introduce new risks for medication safety.

With a study of the use of this technology in the oncology unit of a children’s hospital in Sydney (Australia), this presentation will describe and discuss the challenges of operating on the basis of ultra-safe models of safety; how adherence to protocol and management of risks is sometimes achieved through collective mindfulness practices typical of high-reliability organisations; and the three levels of awareness (situation, team and organisational) that make technology work.
