New applications
All applications must be submitted via Forms for Research Applications (FoRA). Existing applications
All applications must be managed via Forms for Research Applications (FoRA). For more information about this system and the management of existing applications, (e.g. annual reporting, amendment requests) please refer to the Human Research Ethics Management support page.
Fee schedule for commercially sponsored and external research
Fees will be charged for ethics review by a Macquarie University HREC for:
- all industry sponsored research
- researchers who have no formal affiliation with Macquarie University, including those who are appointees of Macquarie University Hospital.
Fees for HREC review are incurred upon application submission and due upon invoice issuance post-review. We do not charge fees for applications submitted for ethics review from researchers who are full-time, part-time, or visiting appointees to the University, unless they are engaged in sponsored research.
Fees charged remain at the discretion of the Director, Research Ethics and Integrity.
Consult our Fee schedule for commercially sponsored and external research for the most up-to-date HREC fees applicable for the review of your research. Please note that these fees are subject to change without notice.
Participant Information and Consent Forms (PICFs)
Several PICFs are available for research projects conducted at Macquarie University and Macquarie University Hospital. If your research involves the use of patient data where patients have consented for the use of their data research purposes, you should include an example of this consent form with your application form.

DIVERSITY AND INCLUSION - Collecting diversity demographic data
This document outlines the wording we would recommend for diversity demographic questions based on current expert advice. These could be included in data systems, forms, and surveys. It may not be necessary to include all these questions –it is always important to consider why you are collecting data and how it will be used.
